Jump to the Vaccine Permission Packet: LINK

May 12, 2021

Dear RSU 1 Families,

You are receiving this letter because you have one or more students aged 12 and above attending RSU 1 schools.  Since yesterday’s approval of the COVID-19 vaccine for children ages 12 and above, RSU 1 has partnered with Mid Coast Hospital to offer a free and optional COVID-19 vaccine clinic for our students.

Our efforts to do this with such an urgent turnaround time are motivated by:

  1. Wanting to ensure transportation and access don’t impede a student’s ability to get vaccinated should they wish to;

  2. Having fewer students miss school in the future as those who get vaccinated do NOT need to quarantine; and

  3. Guaranteeing that we have enough days left in the school calendar to have the second vaccine clinic while RSU 1 is still in session.

If you are interested in your student/s participating in this optional clinic, please review the following details and send back the required forms to your student’s school secretary by FRIDAY, MAY 14th at 3 pm.

Pfizer vaccine clinic dates:

  • May 19th - vaccine #1

  • June 9th - vaccine #2

Clinic hours and location for both May 19th and June 9th:

  • Morse High School/ Bath Tech

  • 9:30 am - 12:30 pm

Transportation to the clinic:

  • For WCS and BMS, transportation will be coordinated and provided by RSU 1 during the school day.

  • For MHS students already on campus this day, they will be provided with a scheduled vaccine time.  For those off campus on these Wednesdays, students may choose to either organize a ride to campus on their own or sign up for a ride by indicating the need on the attached permission form

Permission forms to be returned to your student’s school secretary by close of school on May 14th:

If you are interested, please read and fill out the following forms.

  • Please read the vaccine fact sheet, found here: https://www.fda.gov/media/144414/download and the attached informational letter from Mid Coast Hospital.

  • Complete the RSU 1/ Midcoast Hospital vaccine permission packet which includes:

    • RSU 1 Permission Slip and Transportation Request

    • Patient Registration Form

    • Patient Consent Form

    • CDC Pre-Vaccination Checklist

  • Please note:

    • No ID cards are needed for student vaccinations.

    • Whether you have health insurance or not, the vaccine will be free to you.

Again, all forms must be turned into your student’s school secretary by the end of the day on Friday, May 14th.

Sincerely,

Patrick Manuel
Superintendent

LETTER FROM MID COAST HOSPITAL to RSU 1 FAMILIES

May 12, 2021

Dear Parent or Guardian:

With the U.S. FDA and CDC’s approval of the Pfizer COVID-19 vaccine for all ages 12 years and older (an expansion that includes ages 12 - 15), we are pleased to offer this vaccine to your children ages 12 and older. Mid Coast Hospital will be our healthcare partner administering vaccines.

These vaccines are optional for your child. However, we believe there are some important reasons to consider this opportunity.

  • The COVID-19 virus variants are causing widespread infections among young people in Maine. However, the vaccine is effective against the variants.

  • The vaccine will give you some reassurance this summer. As people get together with friends and family, you will know your student is protected from COVID-19.

  • The vaccine will also help prevent your student from transmitting the virus to others.

  • The vaccine was approved after studies show it is safe and effective.

  • It will be easy to get your student vaccinated at the school. Many vaccine clinics and pharmacies do not offer the COVID-19 vaccine that is approved for children.

  • The vaccine is free.

Mid Coast Hospital will also continue their public clinic at Brunswick Parks & Recreation, 220 Neptune Dr. Clinic hours are Monday through Saturday, 7:30 AM – 12:30 PM & 2:30 PM-7:30 PM. Parents are encouraged to visit www.midcoasthealth.com/clinics for clinic details, including how to make an appointment.

Thank you,

Mid Coast Hospital
